Trapped inside a ghoulish recreation of her hometown, Jules and the rest of the prisoners are hunted by Hammer Head -- the same monster who’s haunted Jules’s nightmares for the past twenty years. As they try to stay one step ahead of Hammer Head and unravel the sick secret behind the murderous game of hide-and-seek they’ve been thrust into, it becomes clear: to survive, they may have to sacrifice the one person Hammer Head wants most: Jules.

Hammer Head’s past is revealed. From a mocked teenager to a sledgehammer-wielding killing machine that refuses to stay dead and buried. As Jules and the remaining prisoners try to survive the night, they realize their only salvation may be to break back into the prison responsible for Hammer Head’s latest killing spree.

Back in the relative safety of the prison, Jules and the remaining prisoners learn of Warden Sullivan’s sick and twisted plan: he’s been feeding Hammer Head for years, allowing him to hunt prisoners all in a misguided attempt to keep him satiated. But the system is failing, and Jules is his last Hail Mary. Meanwhile, Hammer Head has made his way into the prison, leaving a river of blood in his wake.
